Data and purposes
Our contact request form asks for your name, phone number, city and general reason for contacting us. We use these details to receive and answer your request by phone or WhatsApp and follow up on that conversation. This consent does not cover marketing. Please do not include clinical information in this form; it belongs in the appropriate care channel.
Corporate agreement inquiries
To respond to corporate eye care inquiries, we also request job title, business email, company name, industry, address, city and number of employees. We use these details only to contact the requester by phone, email or WhatsApp, understand the organization's needs and prepare information about a possible agreement or event. Consent and sending
You must actively tick the unticked consent box. The form prepares a message to Miópica's official WhatsApp account. You review it and choose whether to press Send. Miópica does not receive the data through this form until you send the WhatsApp message. It includes the consent statement and timestamp; the received conversation serves as evidence of consent.
Your rights
You may access, update and correct your information, ask for evidence of consent and how the data has been used, complain to Colombia's Superintendence of Industry and Commerce, and request erasure or withdraw consent where legally applicable.
